2018-2019 Preschool Applications & Required Forms

Class Offerings for the 2018-2019 School Year:

Toddlers (age 1 by 9/1/18) - Monday/Wednesday or Tuesday/Thursday, 9:00am-12:00pm

Twos (age 2 by 9/1/18) - Tuesday/Thursday, Tuesday/Wednesday/Thursday, or Monday/Wednesday/Friday, 9:00am-12:00pm

Threes (age 3 by 9/1/18) - Monday/Wednesday/Friday or Monday-Thursday, 9:00am-12:00pm

Fours (age 4 by 9/1/18) - Monday-Friday, 9:00am-12:00pm or Monday-Thursday, 9:00am-12:00pm.

Kindergarten (age 5 by 9/1/18) - Monday-Friday, 9:00am-1:00pm

Once your child is registered, the following forms are required before he/she begins school.
Emergency Contact and Medical Release Form
Getting to Know You Form
Certificate of Immunization (Form 3231) - This form is provided by your pediatrician.

Additional Required Forms - if needed:
If you indicated a food allergy or medical condition on your child's application, please complete the applicable forms below.
Food Allergy Action Plan
Medical Condition Form
Authorization for Administration of Medication Form

Good to Know!:
Please refer to this document to help your child acquire some of the skills needed for preschool success! This includes our potty training policy. All children entering a Threes class must be fully potty trained.
